Request an Administrative Hearing for Permit/License Suspension or Revocation Appeals
Important Notice: All administrative hearings are held by telephone and virtual meetings.
A written request for a hearing shall be made by the permit holder within fifteen (15) calendar days of receiving the notice.
Failure to request a hearing within 15 calendar days after receipt of the notice shall be deemed a waiver of the right to a hearing.
To request an appeal, send a written request along with supporting documents by mail or email using the instructions below. Include the date when the permit/license suspension or revocation notice was issued:
