CPSP Protocol and Forms

CPSP Forms:

CPSP Application, Initial Assessment/Reassessment Tool, Individulized Care Plan, and boilerplate protocols are available for download from this website by clicking on the "CPSP Forms/Protocols" link or you can contact your IPAs or health plans to obtain copies of these documents. If you do not contract with any Medi-Cal Managed Care health plans or IPAs, contact the LA County CPSP Program office. 

The CPSP Provider Handbook and Steps to Take Guidelines are sent to all providers upon CPSP certification.
